[Mythnettv] Patch for unsubscribe bug

I've tested this and it seems to work correctly here.  It's a patch for
the bug (listed on TODO list) where the programs that were from inactive
subscriptions were still listed in the TODO list as active, yet they
don't download and prevent other programs from being downloaded.

This patch also requires a change in the syntax from 
mythnettv unsubscribe <url>

to
mythnettv unsubscribe <url> <title>

We can't go with just URL as the URL stored in the program table is not
the same as the one in the URL table.  We could go with only the title.
This should work, and I can't think of a case where there would be two
url's for the same title for a particular subscription.

If you take a look at the unsubscribe function in mythnettv, it is now
two db commands that are run.  This may be able to be condensed into a
single command.
